• The Applicant's name is to be shown on the documentation.
• The date of completion must be shown on the documentation.
• Name on documentation to match the person registered (Shortened version of the name is accepted EG Chris for Christopher.
• The certificate must state Certificate of Attainment PileSafe Rig Operators Licence Course (see accepted evidence).
