  • The attendance record, can only be signed off by one of two facilitators; Rupert Caper or Robert Schuller.
  • The attendance record must be dated 20.07.2018 or later. It can not be before this date.
  • All fields must be completed.
  • Also accepted is V/Line Track Force Protection Currency Requirements.

Competency requirements

  • Applicants name must be shown on document and must match the applicants name as it is registered in the system.
  • Please upload Metro's Level 5 - On-track Maintenance Machine Competent Person's Certificate (Safeworking Accreditation Checklist).
  • Evidence must be signed by applicant. 
  • Evidence must be signed by Accreditation Officer (Full name must be listed).  
  • "Re-Accreditation and "Pass" must be ticked on the first page of the evidence.
  • Black & white or colour copy of the document is acceptable. 
  • Document must be clear / legible.
