Competency Requirements

• Name on the document to match the name in the system (shortened versions accepted, e.g. 'Chris' accepted for Christopher)
• Competency name of ASHTAS Operative to be displayed on the document.
• An actual copy of the document is required. Letters are not accepted in lieu of this.
• The date achieved and expiry date must be displayed.
• Training and assessment name to be listed on the document and to contain the following wording – Certificate of Competence. XXX Has successfully completed training and assessment in ASHTAS Operative”.
• Certificate Number must be indicated on the certificate
