Competency Requirements 

  • Name on the document to match the name in the system (shortened versions accepted, e.g. 'Chris' accepted for Christopher)
  • Name of Quantitative Fit Testing to be displayed on the document.
  • An actual copy of the document is required. Letters are not accepted in lieu of this.
  • The date achieved must be displayed.
  • Fit testing name to be listed on the document and to contain the following wording – “Certificate of Completion. XXX Has fulfilled the requirements of quantitative respirator face fit training and testing”.
  • Certificate Number must be indicated on the certificate
