Competency Requirements 

• Name on the document to match the name in the system (shortened versions accepted, e.g. 'Chris' accepted for Christopher)
• Name of Company/Organisation to be displayed on the document.
• An actual copy of the document must be uploaded. Letters or cards will not be accepted. 
• The date of completion of Test must be displayed (Must be current)
• The Manufacturer and type of Mask tested must be listed.
• Serial Number of Testing Machine must be listed
• Test must reflect a pass 
• Next due date must be displayed

Upload Requirements

• Award Date = Test Date must be displayed 
• Expiry Date = Due Date, which is displayed
