Competency Requirements 

• Name on the document to match the name in the system (shortened versions accepted, e.g. 'Chris' accepted for Christopher) 
• Name of Manufacturer to be displayed on the document. 
• An actual copy of the document must be uploaded. Letters or cards will not be accepted.  
• Document must include wording “Authorized Service Centre Certificate. Xxxx has successfully completed xxxx (Manufacturer name) product service, repair, firmware update and calibration training. Training covers XPS, XPi and personal monitor X2.” 
• The date of issue must be displayed.  
• Validity must be displayed and be current.  
• Training Officer Name and Signature must be displayed
