• We can accept evidence in the form of a certificate, a licence / membership card or a letter
• Accepted evidence can be issued from the following countries, Australia, Canada, Japan, Singapore, United Kingdom & United States.

• Black and white OR colour copy of this document is accepted
• Must be clear and legible
