• Applicants name must be displayed on the uploaded evidence
• A signed ARTC Assessor Agreement form should be uploaded (form can be found at http://www.artc.com.au/Content.aspx?p=276)
• Evidence must be signed by all relevant parties on page 2
• Black and white OR colour copy of this document is accepted
• Must be clear and legible
• No expiry date required
