  • An official State/Territory Apprenticeship/Training Plan/contract must be uploaded.
  • The evidence must include all of the following:
    • Qualification showing Formworker/Falseworker
    • Employer Signature
    • Apprentice/Trainee Signature
    • Apprenticeship/Trainee Name (Must match the name on RIW)
    • Employer Trading name
    • Recognised RTO Number
    • Australian Apprentice Support Network Provider

Upload Requirements

  • Award date = Issue date/Start date listed on document

  • Expiry Date = End date/Completion date listed on document
