Transcript / Statement of Results / Academic History Report (accepted in lieu of Statement of Attainment)


Competency requirements

  • Applicants name to be shown on document
  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • Transcripts can be accepted from ASQA, USI or the issuing RTO
  • All transcripts and Statements of Attainment must meet Australian Skills Quality Authority (ASQA) requirements. 

    The codes '500' and 'HSER' should not be listed on either of these documents. Only codes identified under the Australian Qualification Framework (found at www.training.gov.au) are acceptable.

    If a document indicates non-AQF approved codes, the document cannot be used for verification within the RIW system.


Please note; the certified document that has been issued by the Registered Training Organisation (RTO)** must meet the following requirements to be approved:
  • RTO Name, Code and Logo
  • Name of Applicant
  • Awarded AQF qualification by full title (where a qualification is issued)
  • Awarded AQF qualification - Unit name and code - if the qualification hasn't been awarded
  • Units of competency code and name, date completed, and results to show completion
  • Date certificate issued
  • Issuing organisation's seal, corporate identifier, Unique watermark and/or document number
Please note; the certified document that has been issued by ASQA must meet the following requirements to be approved:

  • Name of Applicant

  • Awarded AQF qualification - Unit name and code - if the qualification hasn't been awarded
  • Units of competency code and name, date completed, and results to show completion
  • Date certificate issued
  • Issuing organisation's seal, corporate identifier, Unique watermark and/or document number
  • There is no expiry date on a Statement of Attainment or Transcript.**RTO registration must only have been current at the time of training completion / awarding the document
  • Transcript of units of competency where the document shows units are partially completed is NOT acceptable.
    • If an ASQA transcript does not have an issue date these transcripts will be accepted with the exception of the following units :
      • All First Aid and units
      • All CPR units
      • UETDRMP004 - Perform elevated work platform controlled descent escape or equivalent/legacy
      • UETTDRRF08 - Perform EWP controlled descent escape or equivalent/legacy
      • Refresher UETDRMP005 - Perform elevated work platform rescue or equivalent/legacy
      • Refresher UETDRRF002 Perform EWP Rescue or equivalent/legacy
      • Refresher UETDRMP007 - Perform rescue from a live low voltage panel or equivalent/legacy
      • Refresher UETDRRF004 Perform rescue from a live LV panel or equivalent/legacy
      • Refresher UETDRMP006 - Perform pole top rescue or equivalent/legacy
      • Refresher UETDRRF003 Perform Pole Top Rescue or equivalent/legacy


If the above units are submitted on an ASQA transcript and do not have an issue date, then please contact [email protected] before uploading the certificates and these transcripts will be reviewed on a case-by-case basis for acceptance.


Please note; the certified document that has been issued by USI must meet the following requirements to be approved:
  • RTO Name
  • Name of Applicant
  • Awarded AQF qualification by full title (where a qualification is issued)
  • Awarded AQF qualification - Unit name and code - if the qualification hasn't been awarded
  • Units of competency code and name, date completed, and results to show completion
  • Date certificate issued (Enrolment Period end date to be used as issue/award/completion date)
  • Issuing organisation's seal, corporate identifier, Unique watermark and/or document number
  • There is no expiry date on a Statement of Attainment or Transcript.**RTO registration must only have been current at the time of training completion / awarding the document
  • Transcript of units of competency where the document shows units are partially completed is NOT acceptable

Upload requirements

  • Document must be issued by either the RTO/ASQA/USI
  • Record the Issue date within RIW
  • the Completion date / Training Date - If completion date is listed on the certificate this date is always used.
  • Issue date - If only an issue date is listed please enter this as the award date
  • If there is a 'from' and a 'to' completion date showing an indication of the course being completed over a time frame longer than 1 day, use the 'to' date as the RIW award date.


  • For ASQA certificates without an issue date, the issue date will be entered as 01/01/1900, and where applicable Expiry date will be entered as 01/01/2099
